Health Independence Programs encompass many of the services that deliver health care to support the transition from hospital to home. These services can also prevent the need for a hospital presentation or stay, with some people accessing these services directly from the community.  Whilst components of the HIP programs are located across the Austin, Heidelberg Repatriation and Royal Talbot campuses, the HIP base is the Health and Rehabilitation Centre which is the "one stop shop" centre for these programs.


This position is based at the HRC - Repatriation campus, but will have responsibilities across the health service and will travel between all three campuses and Austin’s community partners when required.
