About the role

The Integrated Services- HARP and Chronic Rehabilitation programs form part of the Health Independence programs (HIP). The criteria for HARP are:

• Chronic rehabilitation and The Hospital Admission Risk Program (HARP) aims to prevent avoidable hospital presentations and admissions.


• Chronic rehab / HARP targets people with complex care needs that frequently use hospitals or are at imminent risk of hospitalisation and could benefit from rehabilitation.


• Health services may have people who present frequently for many different reasons. Chronic Rehabilitation specialises in managing clients with chronic respiratory disease, chronic heart disease and oncology related conditions.


• The HARP service model is based on the Wagner chronic care model and Kaiser Permanente chronic care framework. Chronic Rehab and HARP services meet evidence-based clinical guidelines

An exciting opportunity exists within our Integrated Services- HARP and Chronic Rehabilitation Team for an Allied Health Assistant who is keen to work within a dynamic and passionate team.

This role is responsible for the organisation and provision of comprehensive, professional Allied Health Assistance and assessment, to the Integrated Care HARP and Chronic Rehab Program clients as required under the supervision of a Physiotherapist.

This is a great opportunity to be part of a supportive Integrated Care Program consisting of Specialist Nursing, Allied Health, Pharmacists, Nurse Practitioner and Geriatricians and make a positive difference to patients and their families.

Full training and support will be provided by the integrated services team.

About you 

To be successful in this role you should possess:

Essential

• Completion of Certificate IV (Allied Health Assistant) from a registered training organisation, or recognised equivalent inclusive of relevant discipline modules to advertised role.


• Well-developed verbal & written communication skills to enable effective liaison with multidisciplinary team including effective clinical handover.


• Well-developed clinical competence and sound theoretical knowledge base relevant to the workplace


• The ability to take direction from supervising staff and work effectively in a inter- disciplinary/multidisciplinary team.


• Well-developed interpersonal skills, with a proven ability to work well within a team environment and relate to medical, nursing and Allied Health staff, clients and families.


• Well-developed skills in organisation/time-management, planning and priority setting (including caseload management).


• Client centred, customer focused and culturally sensitive in all interactions.


• Understanding of continuous quality improvement principles and service innovation.


• Awareness of legal and ethical practice requirements.


• Professional attributes of initiative, self-motivation, goal-setting, empathy and tact.


• Well-developed computer literacy.

Desirable

• Minimum 1-years’ experience working within an acute or rehabilitation facility


• Recent experience in rehabilitation group sessions
