Located on the beautiful Great South Coast, South West Healthcare offers a comprehensive range of medical, surgical and psychiatric services. The organisation serves a catchment in excess of 110,000 and comprises of a total of 282 beds and an extensive range of primary and community services. Warrnambool boasts excellent sporting, education (pre-school to university), social and cultural facilities. The city’s award winning foreshore promenade is a 5.7km path that stretches from the breakwater along the coastline to the Hopkins River mouth.

South West Healthcare (SWH) is excited to confirm the implementation of the Victorian Better at Home initiative. The program is designed to support an increase in bed substitution and early intervention models to deliver more healthcare services within patient's homes where appropriate and preferred by the patient, through use of home-delivered and virtual care.

We're motivated to appoint an enthusiastic and multi-skilled candidate to lead this project and advocate for service provision expansion.

About the role

The Project Lead will work within the broader Better at Home project team to implement shared models of care and increase delivery of care at home or closer to home. The initial focus will include increasing delivery of subacute programs in the home, including GEM@home, implementation of remove monitoring to improve early identification of deterioration in chronic conditions and increasing HITH delivery.

This full time position is available for an immediate start and is available until June 2022.

What you bring

The successful applicant will hold qualification/s in Allied Health or Nursing, and be registered with AHPRA or the appropriate body where relevant, as well as relevant experience in the field. The Project Lead will have a demonstrated ability to engage with staff and consumers and have well-developed computer literacy. Change management, conflict resolution and complaint management experience all are essential for the role. The position requires a current Victorian Drivers Licence.

Post graduate qualifications related to health and leadership, previous management experience or project management experience are desirable but not essential for this position.
