About the role

We are seeking a qualified Allied Health Professional or Registered Nurse to join our dedicated HARP team. This temporary part time position is available from 25/09/2023-25/09/2024 and includes working Monday to Friday from 08.00-16.30.  This role involves care coordination for clients who have chronic and complex health conditions. The position requires assessment and assessment to our clients as well as working closely with a multi-disciplinary team.

Skills and experience

Essential:

  • Registered Nurse Division 1 or relevant Allied Health Professional with current practising certificate from AHPRA
  • 2 years or more of continuous experience in acute or community health setting 
  • Previous experience in rehabilitation programs
  • Advanced patient assessment skills 
  • Advanced interpersonal, organisational, problem solving, time management and decision-making skills
  • Demonstrate the ability to work effectively, both independently and as part of a multidisciplinary health team 
  • High level computer skills to fulfil data processing requirements 
  • Current drivers licence

Desirable:

  • Willingness to undertake further studies in Respiratory/Cardiac Management/Chronic Disease Management/ Rehabilitation
  • Previous experience in Care Coordination
  • An understanding of Grampians Health patient management systems
  • Motivational interviewing skills 
  • Knowledge and experience in myagedcare
