We are seeking applications from motivated individuals to join our dedicated Community Options team as a Case Manager. This permanent full or part time position is available for an immediate start and includes working between Monday and Friday. The primary purpose of this position is to assess the suitability and delivery of Case Management Packages to individuals who are at risk of premature or inappropriate admission to residential care or whose quality of life is adversely affected and who may have complex care needs. Days and hours for this position are flexible, school hours and working from home on some days can be negotiated. In your application, please indicate how many hours you're interested in (minimum of 56 hours and maximum of 80 hours per fortnight).
