Monash Health is excited to announce a vacancy in our Monash Community Population Health and Partnerships team. The Clinical and Systems Integration Lead is a 0.5EFT ongoing position, working directly within Monash Health Community to help deliver our community strategy and improve our service.

About The Role

The Clinical and Systems Integration Lead is responsible for the planning and implementation of key strategic priorities within Monash Health Community. This role is responsible for providing project support to Monash Community management and executive arising from community strategy initiatives, with a focus on clinical systems integration and supporting consumer enagagement across Monash Community.

This is a part time ongoing role, working across Monash Health Community sites and projects as required.

You have:

  • highly developed skills in stakeholder engagement and change management and demonstrated experience in the successful delivery of large complex projects

  • strong knowledge of systems and data management, including analysis, design and problem solving

  • the ability to develop effective working relationships at multiple levels including the executive, technical, clinical areas and non-clinical areas

What You Need:

  • Relevant tertiary healthcare qualification in Allied Health or Nursing or Health Administration (with Healthcare experience)

  • Current AHPRA registration (if applicable)

  • Contemporary knowledge of the primary-tertiary care interface and primary care environment

  • Demonstrated knowledge and application of project management, improvement, change management and process improvement and redesign methodologies

  • Experience implementing changes within a health or community care environment

  • High level communication skills, both verbal and written
