Clinical Case Manager



  • Full time permanent (with monthly ADO)

  • Chandler House- Child and Youth Mental Health Service (CYMHS)

  • An opportunity to work in a community team in Fern Tree Gully supporting children and young people who present with serious mental health difficulties.

About the role


The grade 2 clinical case manager provides assessment and treatment to children and young people who present to CYMHS with severe mental health concerns. The case manger works with the patient and thier familiy and the system around them to provide treatment in a collaborative and recovery oriented episode of care. The case manger will be supported by line, clincial and discipline specific supervision, professional development opportunities and will work as part of a multidisciplinary team. The case manager will work systemically to build relationships and capacity of supports in the community to improve mental helath outcomes for children and young people referred to the clinic.


About you 


To be successful in this role you will possess:



  • Previous experienc working with chidlren, young people and thier families in a mental health context.

  • Current registration with AHPRA

  • Post Graduate qualifications in mentla health, family therapy, trauma informed care.

  • Highly developed interpersonal and engagement skills.

  • A strong commitment to high quality care

  • The ability to work as part of a diverse team

  • Excellent communication and organisational skills
