Clinical Nurse Consultant – Chronic Disease

About the role

The Registered Nurse is a valued and collaborative member of the interdisciplinary health care team at Eastern Health and is accountable for coordinating and delivering safe and comprehensive patient centred care while working competently within their scope of practice and in alignment with the Eastern Health and Nursing & Midwifery Board Practice Standards.

Hospital Admission Risk Program (HARP) services aim to prevent avoidable hospital presentations and admissions by implementing time limited care for clients with complex or chronic conditions who frequently attend hospitals or are at imminent risk of Hospitalisation.

The Chronic Disease Clinical Nurse Consultant role provides care coordination across the hospital and community continuum ensuring integrated seamless care. Care is patient centred, based on meeting client’s individual needs which can include physical and mental health, psychosocial and environmental needs. The role targets clients with complex chronic conditions with a particular focus on clients with respiratory and cardiac disease. This includes active participation in the interdisciplinary case conferences for optimal care to the right patient at the right time.

 The Chronic Disease Clinical Nurse Consultant manages a clinical case load of clients and assists in intake and clinical prioritisation to facilitate smooth patient flow into the service.  They will also act as a clinical resource and advisor to the integrated services team and wider Eastern Health service providers and will influence and contribute to transformation of clinical services at program site level, developing, applying and promoting appropriate and innovative models of care. They will provide oversight for clinical quality and risk, service performance and care standards.

The role includes comprehensive clinical assessments, care planning, monitoring and review. Services are goal orientated and time limited with a focus on ensuring adequate processes are in place for a safe transition and discharge according to the clients ongoing needs.

The Chronic Disease Clinical Nurse Consultant will demonstrate a superior consolidated level of competence, demonstrating expertise in clinical decision making, as outlined in the Eastern Health Domains of Practice, and meet the competencies required to work within the Health Independence Programs. They will be expected to be a positive role model, mentor and resource within the team and assist the Team Leaders and Manager to ensure clinical standards are met and that continuous service improvement occurs. They will lead team planning and development within the clinical speciality area and undertake related projects and research development activities to meet specified clinical needs.

About you 

To be successful in this role you will possess:

  • Bachelor of Nursing or other recognised equivalent formal qualification leading to condition free registration in Australia

  • Registration with the Nursing and Midwifery Board of Australian (NMBA) via AHPRA

  • Post Registration or Post Graduate qualification/graduate certificate or diploma in relevant field

  • Current Drivers Licence


 Research publications and presentations at conferences

  • 5 years post graduate experience

  • Demonstrated skills in the management of chronic disease especially in the care coordination space.

  • Previous experience working with psychosocial needs of patients.

  • Expertise in access management around patient flow, triage and clinical prioritisation practices in line with the DHHS Access policy.

  • Previous exposure to working with KPIs.

  • Excellent communication skills: written, verbal and interpersonal with demonstrated ability to collaborate and work as an effective team member at a local and organisational level to deliver organisational outcomes.

  • Demonstrated ability to work autonomously, utilising well developed critical analysis skills to achieve efficiency and effectiveness.

  • Demonstrated ability to lead significant change practice initiatives.

  • Demonstrated negotiation, problem solving and analytical skills.

  • Demonstrated record of achievement of quality service delivery.

  • Membership of relevant specialty Professional Association.

Previous experience working as a Clinical Nurse Consultant or senior clinical lead role
