• Melton Community Health Service - Outer Community Team is a multidisciplinary team comprising of Nurses, Social Workers, Occupational Therapists, Psychologists and Medical staff.


• We work collaboratively to support the mental health needs of individuals with mental illness who reside in the City of Melton and parts of Brimbank


• Our community team is cohesive and supportive, is positively motivated and genuinely is a great place to work!

Your new role:

• Assess and provide recovery focused treatment interventions to consumers with various psychiatric illnesses


• Coordinate medical, psychosocial and legal aspects of care delivery


• Engage with consumers, their family members and other service providers


• Develop and deliver evidence-based practice and appropriate treatment options


• Work in collaboration with your multi-disciplinary mental health team

About You:

• A motivated mental health clinician committed to continued learning and developing of your clinical skills


• Skilled in engaging consumers and family members, developing rapport and following up with community linkages / referrals to specialist services.


• Ability to work on a dynamic team and draw on the experience of your colleagues as well as being confident to work autonomously


• Excellent communication skills, both in verbal and written form.
