About the role
Case Managers provide essential Mental Health support and treatment to consumers, their carers and family in the City of Greater Shepparton and Moira Shire.
The Community Mental Health Clinician must be able to work autonomously and productively with other members of the multi-disciplinary team to maximise team objectives and maintain a high standard of clinical care. The position includes rotating rostered duties in both the Case Management and Acute Response Teams, and the provision of outreach services across the catchment area therefore travel and working across locations may be involved.
Case managers are responsible for monitoring consumers' mental state and risk, liaising with families/ carers and other service providers and co-ordinating medical appointments and reviews.
