About the Program
The Barwon Health Community Nursing Program supports clients and carers in their home environment to achieve optimum level of health independence through the development and delivery of a high quality, cost effective, locally relevant Community Nursing service. We provide Palliative Care (CPC), Post-Acute Care (PAC), Hospital in the Home (HITH), My Aged care (MAC) and Home and Community Care (HACC) services.
Barwon Health Community Nursing is the largest non-metropolitan based service in Victoria. Community Nursing is a government funded program which provides home nursing service. It consists of three integrated teams based at Barwon Health’s Newcomb, Belmont and Corio Health Centres.
About the Role
Are you interested in working in Community Nursing (District Nursing)? If so, we are looking for a dynamic Community Nurse to cover a period of parental leave.
This position includes day, afternoon and week-end shifts. Successful applicants are able to work additional shifts up to 76hrs per fortnight across all 3 sites.
In this role you will be responsible for the provision of patient-focused holistic nursing care to those assigned to you in order to achieve desired clinical outcomes. You will have highly developed interpersonal skills as well as sound communication skills both verbal and written.
As the successful applicant you will need to have current acute or community health experience. Experience and knowledge in the areas of wound care, palliative care, community health care principles (Active Service Model & Social Model of Health) and continence management is essential.
