About the role
The Cardiovascular Unit in Ballarat opened in 2012 providing interventional radiology and cardiology procedures Monday to Friday plus emergency on call 24/7. The service is continually expanding and requires a dedicated evidence-based pre-admission workforce to ensure quality patient outcomes. Successful applicants will responsible for coordination, triage and pre-admission screening for all elective referrals. They will provide patient and carer education including discharge planning by working collaboratively with a broad range of health care professionals both internal and external to Grampians Health Services.
