What You Will Be Doing The CATT Discharge Coordinator (EEN) is a valued and integral member of the Peninsula Health multidisciplinary team demonstrating flexibility in caring for consumers and their carers / families across the organisation. The Discharge Coordinator works competently within their scope of practice to deliver safe, evidence based, recovery oriented care in partnership with consumers and their families, carers and supporters to achieve optimal health outcomes. The Discharge Coordinator works under the direction of the CAT shift leader and the CAT Operational and Clinical Managers. In conjunction with the multidisciplinary staff, the Discharge Coordinator provides optimum care to patients and their families and carers.
What You NeedWe anticipate you will have:• Endorsed Enrolled Nurse with APHRA Registration• Achievement of four out of the ten Advanced Enrolled Nurse Level 3 Competency Standards and: a minimum of five years post registration experience, or post registration qualification relevant to this role (of at least 6 months or 200 hours duration)• Demonstrated commitment to delivering high quality outcomes for consumers• Advanced Mental Health knowledge and skills • Demonstrated understanding of and commitment to the principles of Recovery Orientated practice in Mental Health Care
