The Emergency Care Coordination Team is looking to recruit a Care Coordinator with an Occupational Therapy background to work in the Austin Health Emergency Department.

The role is a clinical role that relies on patient assessment to identify and organise safe discharge plans, for patients with illness or injuries that present to the Austin Emergency Department

We are seeking an experienced Occupational Therapist with preferable hospital inpatient and or community experience and a willingness to problem solve and work autonomously and as a valued member of the Emergency Care Coordination Team.
