Monash Health Community has an exciting opportunity for an Exercise Physiologist to join our Forensic Mental Health in Community Health (FMHICH) program and Youth and Family Team.  This position will be responsible for providing an effective exercise physiology service.

As well as having a qualification as an exercise physiologist, the applicant must have sound communication, organisational and team work skills.

  • Managing and prioritising participants referred to the program.

  • Undertaking comprehensive assessments.

  • Developing and monitoring participants care plans.

  • Developing and facilitating a variety of group exercise programs.

  • Collaborating with members of the participants care team.

  • The role will require the successful applicant to identify, assess, plan and implement effective interventions for participants on a correction order, that  present with complex Mental Health and trauma histories

As a member of the Forensic Mental Health in Community Health (FMHICH) Program you will be required to provide an efficient, high quality and comprehensive service to Adults 18 and up on Corrections orders residing in the City Of Greater Dandenong, Casey and Cardinia areas. The FMHICH Program, is a comprehensive mental health and broader health evidence-informed service.

The aim of the FMHICH Program is to "to assist eligible individuals to access the mental health care they need and enable them to satisfy the requirement of their Mental Health and Treatment Rehabilitation (MHTR) condition while reducing their recidivism risk.”

It is intended that the Program will assist with providing timely and appropriate assessment and treatment to those who require it, and potentially reduce risk to the community.

As a member of the Youth program you will have experience working with youth aged 16 -25, you will have a high level of knowledge in the application of theory to practice, with skills in assessment, intervention, case management and discharge planning.

You will provide a high standard of assessment, treatment and coordination of Exercise Physiology services for vulnerable youth aged 16 -25 years of age, ensuring you facilitate safe, effective, and person

You will function as a member of a dynamic multi-disciplinary team to promote an integrated and holistic service. This post will require you to work in an outreach model of care as well as community health centre based.
