Opportunity to work in - HOPE (Hospital Outreach Post Suicide Attempt Engagement)/ BIT (Brief Intervention Team)
Eastern Health
Eastern Health is one of Melbourne’s largest public health services. We provide a range of emergency, surgical, medical and general healthcare services, including maternity, palliative care, mental health, drug and alcohol, residential care, community health and state-wide specialist services to people and communities that are diverse in culture, age, socio-economic status, population and healthcare needs.
About the role
This position will work flexibly within the Adult Mental Health Service, Hospital Outreach Post-suicide attempt Engagement (HOPE) service and Brief Intervention team (BIT). These teams work, in close collaboration, to provide Solution Focused, brief intervention services to people referred within the defined target groups, according to the agreed BIT & HOPE models of care.
The HOPE Team provides support for people and those important to them, that are identified as at-risk of suicide and/or repeated intentional self-harm. The team aims to support people in creating change and subsequently alter factors that may have contributed to any suicidal distress, thoughts or attempt. The team supports consumers to progress towards their desired outcomes. In alignment with HOPE, the Brief Intervention team works in partnership with the person and those important to them and other service providers in order to provide high quality clinical mental health assessment, brief intervention, consultation and community linkage services within a recovery oriented framework.
The Family Support Worker, in collaboration with other staff, will utilise relevant and evidence based models of suicide intervention and post-vention, primarily Solution Focused Brief Therapy and the Collaborative Recovery Model to:
• Offer family support work with families, carers and people important to those referred to HOPE/BIT.
• Support the referred person’s carers and families to identify and work towards desired outcomes that reduce the risk of suicide attempt/ suicide and support the person's mental health recovery and wellbeing.
• Work in partnership with people important to the consumer, their carers and families to provide high quality support and linkages in order to promote family wellbeing and improved outcomes for referred consumers and their family/ carer supports
• Utilise the principles and practice of the Solution Focused approach, recovery and advocacy to include the person’s family/ carers in linkages to community support services for ongoing support, care and/ or treatment.
• The position will sit within the HOPE/BIT stream of services, offering enhanced family sensitive and responsive practice in collaboration with other key stakeholders.
