About the role

Community Health is part of the Speciality Medicine and Ambulatory Care program within Eastern Health.

We provide primary care services to the Yarra Valley communities in the Shire of Yarra Ranges including:

  • Adult and Paediatric Allied Health Services.

  • Community Health Nursing, including Women’s Health and Diabetes Education.

  • Counselling (Generic and Family Violence)

  • Social support groups for older people and carers.

  • Health Promotion Groups and Programs, including exercise groups, smoking cessation and diabetes management.

  • General Practice medicine.

We also provide specialist Community Health services in the outer eastern metropolitan region including:

  • Paediatric Allied Health (Maroondah Hospital)

  • Exercise Groups, Adult Physiotherapy and Dietetics (Angliss Hospital)

Your duties would include:


  • To liaise with clients, families, carers, doctors, nurses, and other service providers to provide evidence based physiotherapy

  • To provide a range of physiotherapy services to clients in the centre or in their homes as deemed appropriate, including restorative, reablement and aged care clients. Majority of caseload is centre based physiotherapy management of individuals as well as group based interventions (eg older adults strengthening exercise group, Good Life with Osteoarthritis: Denmark in Australia (GLA:D), postnatal exercise).

  • Involvement in the running of established or new treatment groups

  • To participate in community programs as the need arises.

  • To maintain confidential records of all clients attended in the community setting.


  • To provide statistical data/ service reports to the Health & Ageing Team Leader as requested.

  • To contribute to the sharing of physiotherapy skills and resources of physiotherapy within both the Community Health Service and wider Eastern Health

  • To contribute to the health promotion planning and activities of the Community Health Service Identification of client needs for support and education

  • Development/program planning in conjunction with Community Health Service members’ group program.

  • Involvement in the undergraduate physiotherapy program as a member of the Eastern Health La Trobe Physiotherapy Clinical School.

About you 

To be successful in this role you will possess:

  • Recognised tertiary qualification in Physiotherapy.

  • Current registration as a Physiotherapist with the Australian Health Practitioner Regulation Agency.

  • Eligibility for practicing membership of Australian Physiotherapy Association.

  • Advanced clinical skills in managing a complex caseload using a client-centred approach within the specific clinical area.

  • Highly developed interpersonal and leadership experience

  • A strong commitment to high quality care

  • The ability to work as part of a diverse team

  • Excellent communication and organisational skills

  • Ability to work independently and collaboratively in a multi-disciplinary team

  • Demonstrated capacity to show leadership in a multidisciplinary team.

  • Ability to establish a rapport with clients.

  • Experience in supervision of students and staff.

  • Demonstrate a commitment to maintaining up to date clinical knowledge through professional development and literature.

  • Demonstrated ability to introduce innovative practice.Commitment to improving own practice and undertaking quality and research activities.

  • Demonstrated understanding of professional codes of conduct.

  • Sound computer skills.

  • Previous experience in facilitating and leading a variety of exercise groups
