Acute Hospital – General Medicine

The purpose of this role is for a Grade 2 social worker to provide quality care to clients through competent clinical skills, effective communication, and active participation within a multidisciplinary team environment. Social workers aim to provide comprehensive assessment to enable a targeted intervention plan to be developed in consultation with the patient, their family, carers and significant others. The Social Work team consists of over 40 staff members who work across the City and Royal Park Campuses in acute, sub-acute and community based services across a range of specialties.

Relocation support is available to interstate and international applicants.

Your new role:

  • Complete psychosocial assessments and provide emotional support and counselling to patients and their families and/or carers.
  • Liaise with aged care facilities and service providers and refer to Community Services, Aged Care Assessment Service and Transition Care Program.
  • You will be supported by Social Work team leaders (grade 3) and clinical lead (grade 4) on site.
  • You will help us achieve excellence in patient care, primarily in general medicine.

About you:

  • You are a qualified Social Worker (either current member or current eligibility for AASW).
  • You are a quick learner with proven ability to work autonomously in hospital environments.
  • You are able to deliver excellent consumer focussed clinical care to patients and their carers.
  • You have the experience and skills to mentor and supervise junior staff.
