An exciting opportunity exists for an experienced Allied Health Assistant to work on a temporary part time basis (0.7 EFT) within the RAH (Rapid Allied Health) and the ACE ED Care Coordination teams at Western Health.
About the Unit
The Rapid Allied Health (RAH) at home service aims to prevent avoidable hospital admissions and presentations to ED by providing a rapid response and short term Allied Health Intervention. RAH accepts referrals from (but not limited to): Emergency Department Care Coordinators, Internal WH wards, External Hospitals, General Practitioners, Medical specialists, and Ambulance Victoria. The RAH team consists of occupational therapist, physiotherapists and social workers and is integrated with the ACE ED Care Coordination team.
About the Role
This role will involve providing allied health assistant intervention in patients’ homes to support safe discharge or to prevent hospitalization. The caseload is varied and will include patients with acute and chronic presentations, often on the background of complex psychosocial situations. We are seeking an applicant who enjoys working in dynamic teams with an ever changing and fast paced environment and is committed to providing best patient care.
