About the Role

Monash Watch aims to keep some of our most vulnerable patients, especially those with complex chronic health issues, healthier at home. In so doing, we aim to reduce avoidable hospitalisations and Emergency Department presentations. Monash Watch will regularly call patients to conduct self-rated health checks, when a threat to a patient’s homeostasis occurs the Health Coach will be alerted to investigate and intervene. Based on our success to date, the service has expanded to cover a larger cohort of patients. The position is for 6 months and has the possibility of extension pending evaluation and delivery outcomes.

We currently have 1 full time and 1 part time (0.8 EFT) position available for a dynamic and experienced Allied Health Professional or Registered Nurse. The successful candidate will be adaptive, have exceptional communication skills, be culturally sensitive and enjoy working within a small multidisciplinary team. The patients are diverse and often complex, there will be a strong focus on service development and patient centred care.  The role provides an excellent opportunity for personal and professional growth.

About You

The Health Coach helps manage risks associated with avoidable hospitalisations, and builds trust and relationships, bridging boundaries between services so they appear to be integrated to the patients. They support clinical management within scope of practice, promote self-management and support their patients to navigate relevant services. The Health Coach works effectively and collaboratively with other service providers including the patient’s GP.

If you are a suitably qualified and experienced clinician who is patient centred and enjoys direct patient contact, both face to face and on the phone, then this is the role for you. You will be able to use your aptitude for supporting chronic, complex and aged related health issues with patients and providers, and enjoy enhancing the patient experience.
