The Intensive Community Treatment team currently has a full time position available for a Grade 2 Social Worker or Occupational Therapist. Our team has recently expanded and are incorporating extended hours which will include afternoon shift.

Our Journey:

The ICT Team is a newly expanded innovative team comprising of Nurses, Social Worker, OT’s, Psychiatrists and Registrars, we are dedicated to delivery of high quality, patient centred care in the community as an alternative to hospitalisation.

As a clinician in the ICT program you will offer home based mental health care as an alternative to treatment in an Older Adult Mental Health (OAMHS) inpatient setting, with the aim being primarily in preventing hospitalisation. You will have regard for the expressed wish of both the older person and family or carer as you work collaboratively to minimise the length of stay in an inpatient unit and work as a team to provide intensive community treatment during an acute episode of illness.

To achieve the goals of ICT you will work closely with psychosocial support providers and residential support programs, including local council or non-government providers, to provide holistic care to older people, delivering time limited intensive support to the person in their home context.

  • Experience in acute mental health settings or equivalent
  • Older Adult Mental Health experience highly regarded

  • Relevant qualifications (aligned to position description)
  • Demonstrated experience
  • Current AHPRA registration (OT only) or Eligibility for membership with AASW (SW only)
  • Include any other mandatory/required skills or licences
  • Australian working rights
  • Current Victorian Driver’s Licence
