About the role

Grampians Health is looking for Health Care Workers to join our community support team at the HUB.

This role would provide high-quality in-home services including respite, personal care and domestic assistance to eligible community members.

We are looking for people with a desire to help others, and who enjoy variety in their roles. This position will work closely with other team members to ensure the best client outcomes and holistic care. Training is available on the job, and the role provides a high level of flexibility with hours and days worked.

Key Responsibilities

  • Assist clients with daily living in accordance with care plans
  • Reporting of changes in wellbeing 
  • Provide and assist involvement in meaningful social activities 

Skills and experience

  • Certificate III in Individual Support or equivalent desirable
  • Current Victorian Driver’s License
