Lead Clinician (Case Manager)

As part of the Maryborough Community Mental Health Team, you will provide recovery focused treatment and support to persons aged 16-64 years living in the community who are experiencing mental illnesses. You will remain involved throughout all stages of their recovery by providing assessment, treatment, short-term interventions, referrals and discharge planning.



Classification: 
Registered Nurse (Grade 3), Social Worker (Grade 2) or Occupational Therapist (Grade 2)


Salary: $41 to $51 per hour plus superannuation dependent upon classification and experience


Hours: Up to 80 hours - Full time or part time can be negotiated


Status: Permanent



You will:



  • Make a difference to individuals and their families/carer(s) with mental health challenges

  • Provide assessment and clinical treatment, case management and support for patients, their families and carer(s)

  • Identify, engage and include other key community agencies and service providers involved in the provision of rehabilitation, recovery, support, and discharge planning consistent with the patients and family/carer(s) needs

  • Participate in community consultation, awareness and education activities and develop community resources to increase the understanding of psychiatric disorders and service delivery models in the community

The Team


The Maryborough community based adult mental health service provides recovery focused care Monday to Friday to persons living in their communities experiencing mental illness.


In the context of an integrated community mental health team, and under the direction of the Manager and team Psychiatrist, Community Mental Health Clinicians provide psychiatric triage, intake, assessment and treatment, psycho-education and support for patients of Psychiatric Services and their families /carer(s).
