Monash Health Community is excited to announce a new My Aged Care (MAC) Co-ordinator /Team Leader position within the Community Access Team.



  • The Team Leader / MAC Co-ordinator is an integral role to ensure that referred clients are triaged, registered and booked into an appointment with the most appropriate Service.

  • Lead the functional operations of the My Aged Care portal

  • Work as part of a multi-disciplinary team to enhance a client’s journey through Monash Health Community

What You Need:



  • Relevant Qualification in Allied Health

  • Demonstrated work experience in a leadership position.

  • Current Victorian Driver’s License

  • A working with children check.

  • Australian working rights
