About Kingston Aged Care Assessment Service

The objective of the Kingston Aged Care Assessment Service (ACAS) is to provide a comprehensive, holistic assessment of an older person or a younger person with an age-related needs. The assessment is to identify the resources, strengths and problems of the person and to develop an individualised, coordinated support plan which focuses on client goals and re-ablement. ACAS assesses a person’s eligibility for Commonwealth funded care such as home care packages, restorative care, transition care, respite and residential care

About Transition Care Program

The Transition Care Program (TCP) is a major policy initiative between the State and Commonwealth Governments. The TCP aims to minimise the number of older people experiencing inappropriate extended hospital lengths of stay, being prematurely admitted to residential care and/or prevent unnecessary presentations or admission to the Emergency Department. The program provides short term support and active management for older people at the interface of the acute/subacute and aged care (i.e. Community and residential) sector. The program is goal orientated to optimise the patient’s functional capacity and to finalise and access their longer term care arrangements or maintenance at home.

The TCP can be provided as a bed based or as a home based service. By offering case management, low intensity therapy and personal support, TCP allows older people more time and support in a non-hospital environment to complete their restorative processes, optimise their functional capacity and finalise their long term care arrangements.   

About The Role

We are seeking an experienced, enthusiastic and accomplished clinical leader who is ready to take the next step in their clinical career to join the Sub Acute & Residential Care leadership team. This managerial role will facilitate excellence in care by working with the team to support clients.

About You

You are a dynamic, experienced clinical leader. You are experienced in acute, aged and community care environments, and have an interest or experience in access and flow.  You are also highly organised with well-developed critical thinking and analytical skills. You are passionate about client centred care and able to work collaboratively within a dedicated and compassionate team.

You Are Also:

  • Autonomous & able to lead a team by example to ensure patient safety & clinical best practise

  • Articulate & a clear commutator focused on problem solving, creatively & effectively

  • Supportive & able to drive a team

  • An advocate of Monash Health’s Strategic Guiding Principles

What You Need:

  • Proven post graduate experience

  • Demonstrated operational management experience

  • Current Victorian drivers licence essential

  • Understanding of home based services an advantage
