The Role:

In this role you will be driving the Stawell Regional Health's community services strategy, playing a key part in growing the sub-acute and community-based services to best meet local community needs.

Reporting directly to the Director of Primary and Community Care, you will be responsible for leading the dedicated teams and providing operational oversight to many services and programs, including:

  • Community Nursing: District Nursing, Hospital in the Home, Memory Support Nursing
  • Care Coordination:  Post-Acute Care (PAC), Hospital Admissions Risk Program (HARP), Transition Care Program (TCP), COVID in the Home, and Healthy@Home
  • Social Supports: Social Support Group day program

This role is suited to a nurse with leadership experience in community nursing, or a similar clinical domain; or an aspiring nurse leader looking to take the next step in their career.
