Mental Health Clinician - HOPE Exciting opportunity to be part of a program initiative for suicide prevention - Hospital Outreach Post-suicidal Engagement (HOPE).- OT - Grade 2/ SW Grade 2 / RPN Grade 3  - Position will move to Atherton rd, Oakleigh by mid 2022.  This new hub will provide a diverse range of Mental health services previously offered at a number of different sites and will provide a more tailored service for consumers consistent with recent Royal Commission.

The (HOPE) initiative provides psychosocial support for people who are identified as having presented with a suicidal or self-harm attempt, or suicidal or intentional self-harm ideation. The initiative aims to provide follow-up care and support for people who traditionally would not have received such intervention immediately after hospital discharge, with broader referral pathways and targeted psychosocial support.

About the role

The position requires an individual with skills which will enable them to liaise closely with a number of Inpatient and community agencies and work as part of a multidisciplinary team to ensure best practice care is provided. This will ensure a comprehensive assessment, collaborative treatment design and appropriate referral of consumers presenting to the team.Demonstrating your expertise in mental Health and clinical skills, you will undertake clinical mental state and risk assessments and crisis management. Client contact may occur in hospital, via teleconference or at their own accommodation as required.


As a clinical role, you will provide bio-psycho-social assessment of consumers and deliver clinical treatment and care to resolve or ameliorate emotional distress with consumers engaged in the HOPE program.Mental Health clinicians are required to provide the best care to our communities.
