The Opportunity

We require a Mental Health Nurse, Occupational Therapist or Social Worker to join our Continuing Care team at Wodonga Adult Community Mental Health. As part of the continuing care team you will provide case management to adult clients acessng our service with a recovery orientated approach. Primary responsibilities include case management, monitoring mental state and risk, liaising with support services, participating in multidiciplinary clinical reviews along with psychiatry appointments. You will also collaboratively develop, implement and review individual recovery plans with targeted strategies to meet psychological and social needs of individuals, their families and/or support networks.
