Classification: RN G3A Y1
Salary per hour (exc. super): $50.32

Role Description
As a Registered Nurse Case Manager (Care Coordinator), you will provide effective and high quality care coordination specifically to our Home Care Package and NDIS clients, and focus on enhancing and supporting independence in everyday life to enable them to continue to live in their own home environment.
You will complete thorough assessments, including consideration of risks to the consumer’s health and well-being, which will inform the delivery of safe and effective care and services and identify all the risks associated with a consumer’s environmental, physical, mental or emotional condition or capacity.
You will use local knowledge of other community services, enabling you to effectively collaborate with clients to understand their care goals, develop care plans to meet their needs and liaise with service providers to adjust services as needed.
