About the role
This is an exciting new Practitioner role working in a pilot model, where you will provide specialised nursing care to community dwelling patients including patients with chronic lung disease and pneumonia. The new project aims to reduce inpatient length of stay and hospital admission risk for this patient group, utilising an early discharge collaborative model of care in partnership with HITH.
On a daily basis you will live our Eastern Health values as you apply and lead innovative strategies & deliver great care in the specialised nursing management of clients with chronic lung disease.
You will be expected to possess clinical expertise and relevant depth of experience, specifically in regard to self-management and coaching strategies. You will have excellent communication & advocacy skills; great initiative; love challenge, growth and change; and possess the proven ability to actively collaborate, lead & deliver work to improve outcomes & experiences for your patients and teams.
