The Role


The focus of this role is to implement a model of care where people living is residential care are offered an alternative care pathway as hospital avoidance.  The predicted role is being developed and will mature over time.  It is predicted to include:



  • Support aged appropriate treatment of chronic conditions.

  • Support falls management.

  • Support uncomplicated behaviour management related to cognitive impairment.

  • Support sick patient management including respiratory conditions. 


About You


You are a dynamic Nurse Practitioner/ nurse practitioner candidate with a passion for the care of older persons, especially those with a significant level of frailty requiring care in a residential environment.


You value a person-centred care framework that respects the rights of the person to make informed decisions throughout their health/illness experience and provide accurate and appropriately interpreted information about health assessment findings and/or diagnoses, including outcomes and prognosis. When appropriate you support significant others in the patients care.


With a proven ability to work both independently and collaboratively, you readily build strong relationships with stakeholders in the delivery of client centred care.


Looking after Our People:



  • We strive to ensure that all Monash Health work in accordance with all relevant Occupational Health and Safety, Infection Prevention & Respiratory Protection Program procedures

  • Ensuring you work within the scope of practice as a Nurse Practitioner

  • Completing all mandatory and competency assessment training 


What You Need


Essential:



  • Demonstrated recent experience as a Nurse Practitioner or in an advanced practice role with an older persons focus

  • Current AHPRA registration

  • Endorsement as a Nurse Practitioner or enrolled in Masters of Nursing Nurse Practitioner

  • Australian working rights

  • Valid Driver's licence

  • Current Working with Children’s Check

  • Current Police Check


Desirable:



  • Previous Community Nursing experience

  • Post-graduate course in Aged Care related courses highly regarded
