Nurse Unit Manager - Health Independence Program

Located in Gippsland, Latrobe Regional Hospital (LRH) is seeking to appoint an experienced nurse as Nurse Unit Manager to lead and manage the successful delivery of the Health Independence Program.

The Health Independence Program (HIP) provides patients with healthcare services to support their transition between hospital and home and preventing hospital presentations and admissions.

The programs provided include:

  • Latrobe Post-Acute Care (PAC) Program
  • Hospital Admission Risk Program (HARP)
  • Subacute Ambulatory Care Services (SACS), including Community Rehabilitation and Specialist Clinics
  • Transition Care Program (TCP)
  • Residential in Reach Program (RiR)

The HIP Nurse Unit Manager is responsible for the operational performance of the programs and managing a multidisciplinary team.  We’re looking for applicants with strong leadership and management experience who are driven and passionate about delivering high-quality patient-centre care, with the ability to implement effective solutions to overcome challenges.

Our ideal candidate will have:

  • Minimum of 5 years clinical experience
  • Demonstrated leadership and management experience, including financial and human resource management
  • High level decision-making ability
  • A passion delivering best practice in the delivery of health care services
  • Demonstrated ability to introduce and manage change
  • Demonstrated proficiency in written and oral communication including experience in analysing data, writing reports and making recommendations
