About The Hospital Admission Risk Program (HARP) & Chronic Rehabilitation Program:


The Integrated Care HARP and Chronic Rehab programs form part of the Health Independence programs (HIP). The criteria for HARP are:



  • HARP aims to prevent avoidable hospital presentations and admissions.

  • HARP targets people with complex care needs that frequently use hospitals or are at imminent risk of hospitalisation and could benefit from coordinated care. This may include people with a chronic disease, conditions of ageing or psychosocial complex needs.

  • Health services may have people who present frequently for many different reasons. HARP specialises in managing people with diabetes, chronic respiratory disease, and chronic heart disease, complex psychosocial and complex aged needs.

  • HARP provides specialist medical care and multidisciplinary services through an integrated response of hospital and community services, delivered in hospital, community and home-based settings.

  • The HARP service model is based on the Wagner chronic care model and Kaiser Permanente chronic care framework.  HARP services meet evidence-based clinical guidelines.

  • HARP works in collaboration with other acute, community, aged care and specialist services, but does not duplicate them. 


About the role


An exciting opportunity exists within our Integrated Care HARP and Chronic Rehab Team for a Grade 2 Occupational Therapist who is keen to work within a dynamic and passionate team.


This Grade 2 Occupational Therapist is responsible for the organisation and provision of comprehensive, professional Occupational Therapy assessment, services and Care Coordination to the Integrated Care HARP and Chronic Rehab Program clients as required.


This is a great opportunity to be part of a supportive Integrated Services Program consisting of Specialist Nursing, Allied Health, Pharmacists, Nurse Practitioner and Geriatricians and make a positive difference to patients and their families.


About you 


To be successful in this role you should possess:



  • A degree (or equivalent) in Occupational Therapy

  • Membership of Occupational Therapy Board of Australia via Australian Health Practitioner Regulation Agency (AHPRA)

  • Current registration with AHPRA

  • Special interest and experience in working with older adults

  • Current Victorian Driver’s Licence

  • Advanced clinical skills in managing a complex caseload using a client-centred approach

  • High level communication skills, both written and oral

  • Demonstrated capacity to work / show leadership in a multidisciplinary team

  • Experience in supervision of students and staff

  • Commitment to improving own practice and undertaking quality and research activities

  • Demonstrated understanding of professional codes of conduct

  • Sound computer skills

  • Highly developed interpersonal and leadership experience

  • A strong commitment to high quality care

  • The ability to work as part of a diverse team

  • Excellent communication and organisational skills
