An exciting opportunity for a Grade 2 Casual & weekend Physiotherapist to join the Better@Home Sube Acute Team.

Better@home is an inpatient model and part of a 2020-2021 State Budget initiative which aims to deliver more healthcare within patients’ homes where appropriate and supported by the patient, through use of home-delivered and virtual care. Austin Health’s Better@home-subacute program operates 7 days per week and sits within the Continuing Care Division and includes GEM@home, Rehab@home and Residential In Reach.

The casual Grade 2 Physiotherapist is a member of the Better@home-subacute team. The casual Grade 2 Physiotherapist works in close liaison with the Better@home team to respond to the changing clinical needs of patients as they progress through Better@home services through to community-based care. The casual Grade 2 Physiotherapist will work as part of the multidisciplinary team and undertake physiotherapy assessment and management of patients at home, in the centre or in the community to optimise or maintain independence and functional capacity as care needs change and in accordance with the principles of goal-based, patient-centred care.
