About the role

South West Healthcare's (SWH) Community Rehabilitation Department is seeking a motivated and experienced Registered Nurse to join our team as a part time Rapid Assessment Community Evaluation (RACE) Nurse. This role will work collaboratively in a multi-disciplinary team to provide rapid specialist nursing assessment of patients with treating physician, to reduce avoidable presentation to the Emergency Department or hospital admission. In the position you will maintain a high quality care environment and promote and participate in quality improvement activities and initiatives.

Part time hours available and applicants must be available to work a rotating roster to cover weekend shifts.

What you bring

As a Registered Nurse you will have experience and skills delivering accurate clinical assessment and consultancy, care planning and treatment to community clients. Working as part of a collaborative and highly skilled team of professionals, excellent verbal and written communications skills are essential, and a comprehensive understanding and knowledge of legislation, regulation and contemporary practice is a must. Current registration with AHPRA as a Registered Nurse is essential.
