About the role

This West Hume Region Clinical Nurse Consultant (CNC) is an advanced Clinical leadership Position and will provide a capacity building role, including a clinical consultancy service for the West Hume Region District Nursing services. This advanced practice role functions to provide a safe, quality and cost effective, positive person centred health experience for individuals and services across the West Hume Region within the constraints and accountability of the Commonwealth/State Home Support Funding Models and reporting.

The CNC conducts advanced comprehensive and holistic health assessments and adopts evidence-based practice to deliver care to clients within our community.

The CNC engages the clients and regional services to customise care to meet client needs and improves health outcomes. 

This is an opportunity to influence and lead innovation in how wound care is delivered in the community setting.   

We are looking for

  • Current registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse.
  • Minimum of five years’ experience with a broad-based general nursing experience across a variety of practice settings, with a scope of practice to encompass Continencemanagement.
  • Graduate Diploma or Graduate Certificate in Continence management or related field.
  • Commitment to enrol (or completion of) a Nursing and Midwifery Board of Australia approved program of study at Master’s level or equivalent.  
  • Leader of Nursing

Your Check will be assessed to determine your eligibility to work within a NDIS role.  If you do not receive clearance from the NDIS Worker Screening Unit or you do not agree to participate in the Scheme you cannot be engaged by a NDIS provider. Appointments to this role are also subject to satisfactory Victorian ‘Employee’ Working with Children’s Checks. Completion of a Commonwealth of Australia Statutory Declaration is required prior to commencement.
