What You Will Be Doing

This role is part of the Better At Home subacute strategy, a body of work aimed at providing alternatives to bed-based care and reducing the need for hospital presentation, whilst improving the quality of care and outcomes for patients. Better At Home subacute aims to provide as much care as is safely possible, outside a bed-based environment by broadening the scope of its home-based services, addressing service gaps and enhancing and integrating these services to result in a more seamless response for patients, their families and carers. What You Need

We anticipate you will have:
• Demonstrated ability to co-ordinate patient care, set priorities and work independently.
• Willingness to participate in ongoing clinical rehabilitation education.
• Ability to work as part of a multi-disciplinary team.
• Able to use basic computer software applications.
• Able to demonstrate ongoing commitment to professional development.
• Takes responsibility for own actions and is proactive and self-starting.
• Is able to identify continuous improvement opportunities for improved patient/consumer outcomes.
