The Opportunity

We require a dynamic Credentialled Diabetes Educator to join our Community Health cross campus team for a permanent part time (0.6 FTE) position. (Clinical Nurse Specialist 2/Registered Nurse 4A equivalent)   Primary responsibilities include:

- To provide Diabetes Edcuation services including clinical, educational and consulative intervention, in line with AWH objectives

- To provide a high standard of clinical care for clients within our community with a range of diabetes diagnosis, including gestational diabetes, Type 1 and Type 2 diabetes and Paediatric diabetes management.  This will require use of a range of diabetes technologies including insulin pumps and continuous glucose monitoring devices.

- Provide Diabetes Education services in a range of setting which may include outpatient services, inpatient services and outreach clinics.
