Our Journey:

About Monash Health in the Community

Helping people live their best lives.

As Monash Health in the Community, we actively engage with clients across the age spectrum to improve and maintain health, independence and well-being.  We do this by coordinating and/or providing integrated, multidisciplinary care across multiple settings within the local community. Our aim is to help clients live their best lives by enabling them to access the support they need, when they need it.

Infant, Child and Family Health and?Wellbeing Hub

As one of the key recommendations from the Royal Commission into Victoria’s Mental Health System and the National Children’s Mental Health and Wellbeing Strategy, Monash Health was identified as one of three lead providers to roll out this initiative to support infants, children and families in the South Eastern region of Victoria.  Designed as one central location for mental health, health and wellbeing support for infants, children and families we provide support to individuals from 0-11 years experiencing developmental, behavioural and emotional challenges through expertise from a range of specialist medical and allied health services, including paediatricians, occupational therapists, psychologists and speech pathologists and offer parenting supports.  The Infant, Child and Family Health and Wellbeing Hub will sit alongside ELHMS - Monash Childrens and the Child and Family Program - Monash Health Community.

A Day in the Life:

You will work with children aged up to and including 11 years and their families, experiencing one or more of the following: developmental, emotional, relational or behavioural challenges, to support healthy child development and supportive family relationships and environments.

As a member of the Infant, Child and Family Health and Wellbeing Hub (the Hub) commit to providing high quality nursing services, including assessment and treatment, to children and their families living in the City of Greater Dandenong, City of Casey and the Shire of Cardinia. The focus of the position is to provide evidence-based, best practice assessments and interventions for a diverse range of issues experienced by consumers in the service.

As this is a new service, as part of the recommendations of the Mental Health Royal Commission, there will be two phases of service delivery. The first phase will be an upstanding of an interim model of care taken from the needs of both existing mental health and community health paediatric services within Monash Health that meet the objectives of the Hub. The second phase will run in parallel to phase one and will consist of a new codesigned model of care for the ongoing service delivery of the Hub. Within the first 12 months there will be time allocated to workshopping the new model of care with other team members, consumers, and stakeholders.

This role is based at Cranbourne, however you may need to work at other Monash Health locations from time to time.

You will Bring:

  • Genuine desire to work in a multidisciplinary assessment and treatment group setting
