About South 1 GEM at Monash Health
The Sub Acute and Residential Service program is very excited to be opening a new sub-acute ward at The Kingston Centre (Cheltenham). South 1 will be a Geriatric Evaluation Management (GEM) ward providing specialist assessment, treatment, and management of patients with complex and multi-dimensional needs including medical, functional, and often cognitive conditions. The ward will be undertaking capital works to ensure that this space is fit for purpose to allow the multidisciplinary to delivery the highest quality care. The Sub Acute and Residential Service program dedicated team to delivering patient care in line with our iCare values and ensure that all patients are treated with dignity and respect while in our care.
About the Role
We currently have vacancies within ward – South 1 for Registered Nurses, each to work through a rotating roster consisting of 7 days, all shifts, amongst our dedicated team.
The capacity of work to undertake complex discharge planning, complex wounds, patient training, address continence and catheter issues. With the overall aim to see that all patients are treated with dignity and respect as they transition into longer term Residential care or back into their home environment.
About You
Understanding the importance of patient centred care, you have experience within an acute/subacute hospital ward and have a genuine interest in working and interacting with our diverse patients who, at times, can demonstrate challenging behaviours and complex needs. Your critical thinking, willingness to learn and ability to work cohesively and share knowledge within a team will see you excel in this role whilst we provide you with guidance, support and training to learn the rhythm of rehabilitation.
