This position is at operational and will be responsible for the delivery of Renal Dialysis to patients of CHRH.

Key Responsibilities, Duties and Performance Indicators

Key Responsibilities

  • Promote and deliver a high standard of person-centred care in the Renal Dialysis unit.
  • Provide accurate clinical information at handovers and meetings.
  • Establish a relationship between the patient and yourself in order to achieve the best possible outcomes for your patient.
  • Support patient care in accordance with the best interest of the patient.
  • Apply knowledge and skills to undertake systematic assessments of patients, develop and document the plan of care.
  • Recognise a change in the patient’s condition, in particular deterioration, take necessary action and document variation in the care plan.
  • To work in a highly organised manner with the ability to balance competing priorities and work demands and to identify and respond appropriately to critical and urgent clinical needs.
  • Establish and maintain effective interpersonal relationships with other members of the team.
  • To understand and operate in a work environment driven by the need to meet clinical quality and safety guidelines: and
  • To work closely and in collaboration with internal and external stokeholds.

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30th November 2019.  Each of our campuses retains the name of the township in which they are situated - Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
