Senior Clinician/ Senior Psychiatric Nurse - Clozapine GP Shared Care

About the role
The Eastern Health Mental Health and Wellbeing Program has a Clozapine GP Shared Care team that works working within the Brief Intervention Team (BIT) and Hospital Outreach Post- Suicidal Engagement (HOPE) teams. The role requires an experienced and highly skilled mental health professional.
The Senior Clinician - Clozapine GP Shared Care works with the treating Consultant Psychiatrist, providing consumers with a consultation on a six monthly basis, with subsequent recommendations being forwarded to the treating General Practitioner.
The position of Senior Clinician-Clozapine GP Shared Care will be responsible for the coordination of Clozapine related matters and monitoring across the Eastern Health Mental Health and Wellbeing Program (MHWP), at all times ensuring compliance with set protocols, guidelines, policies and procedures. The role works collaboratively with all Eastern Health services, with consumers of the service and their families, community General Practitioners and with approved community dispensing pharmacies.
In addition, for part of this role the senior clinician will work, outside of the Clozapine Share Care team, as part of the broader BIT & HOPE teams providing comprehensive recovery orientated assessment and therapy. The senior clinician will work collaboratively with consumers to strive towards their preferred future whilst working collaboratively with their families, carers and relevant agencies. The BIT and HOPE teams practice Solution-focused recovery care through the lens of the Collaborative Recovery Model.
