Senior Clinician Allied Health or Nursing - CRP Rapid Triage Team

  • Part time fixed term until 30 June 2026
  • 30.4 hours per week (4 days per week or worked over 5 days)
  • Ideally suited for Physiotherapists, Occupational therapists, Social Worker or Nursing staff
  • Join the Community Rehabilitation Program (CRP) Rapid Face to Face Triage Team for clients discharged from Eastern Health (EH) Acute and Subacute wards to home

About the Role

You will assist with rapid triage, assessment, goal setting and discharge planning for clients referred to CRP from EH Acute and Subacute wards. You will recognise the deteriorating patient and escalate to medical staff for management to prevent avoidable readmissions to hospital. Partake in holistic discharge planning following inpatient stay, including liaising with community nursing, Post-Acute Care, HARP co-ordinators and allied health.

Liaise with CRP allied health staff, medical staff and other stakeholders to develop a plan of care tailored to meet the need of individual patients which addresses relevant clinical needs

Subsequent follow-up will be delivered via a hybrid model of telehealth, face-to-face and home visits, as clinically indicated.

Strong interpersonal and communication skills will enable you to thrive in this role. The team will work closely together and be strong advocates for this vulnerable and frail client group. The team will use a mix of modes such as face to face interventions and telehealth.

About you

To be successful in this role you will possess:

  • Demonstrated expertise in managing complex and vulnerable, frail populations, initiating appropriate therapies and supportive treatments.
  • Demonstrate a high level of clinical reasoning and advanced casework skills, using a patient-centred approach within multi-dimensional assessments, formulating appropriately targeted treatment plans with realistic achievable goals.
  • Develop and implement treatment plans in collaboration with patients and significant carers.
  • Collaboration with other stakeholders and especially referrers, CRP and Post-Acute Care teams.
  • Demonstrated ability to participate in innovative practice and quality improvement.
  • Deliver a high quality, accessible and coordinated service to our clients both in the client’s home and in the centre.
  • Be an active member of the interdisciplinary team.
  • Demonstrate high-level organisational skills through efficient time management and professional responsibility including modelling the ability to appropriately prioritise workload and manage multiple competing demands.
