About the role
A new and exciting opportunity has been developed for an enthusiastic and motivated Grade 3 Social Worker who is keen to work within a dynamic and growing team that promotes best practice and great patient outcomes in all clinical areas.
This position is responsible for providing social work services within the expanding Oncology in the Home Program. Additionally, this role requires supportive care screening and planning for patients who are currently being treated in the Day Oncology Medical Unit as they transition to Oncology in the Home. The senior social worker will also be expected to support the Nurse Unit Manager, in providing allied health leadership and assisting in the introduction and development of allied health specific workflows and models of care.
The senior social worker will demonstrate advanced skills in psychosocial assessments, risk assessments, complex discharge planning and social work interventions promoting optimum patient outcomes, best practice standards and clinical excellence. This position will also deliver professional support, supervision and multidisciplinary leadership and will be regarded as an essential member of the broader nursing and medical team.
