Monash Health is offering an exciting leadership opportunity in the (HOPE) Hospital Outreach Post-suicidal Engagement program.  Hope contributes to an initiative that aims to provide follow-up care and support for people who traditionally would not have received such intervention immediately after hospital discharge, with broad referral pathways and targeted psychosocial support.

About The Role

The Monash Adult Mental Health Program Hospital Outreach Post-suicidal Engagement (HOPE) initiative provides psycho-social support for people who are identified as having presented with a suicidal or self-harm attempt, or suicidal or intentional self-harm ideation. The initiative aims to provide follow-up care and support for people who traditionally would not have received such intervention immediately after hospital discharge, with broader referral pathways and targeted psychosocial support. The aim of the initiative is to prevent avoidable suicide attempts and reduce the number of completed suicides by providing targeted psychosocial support services to a cohort of people in an assertive manner in the community for up to 3 months from the time of discharge.

Within the team structure the HOPE Team Leader, together with other senior clinical staff, will ensure that the highest professional standards are maintained and that clients are assessed in a timely fashion. The Team leader will also carry a clinical caseload within the HOPE service up to a maximum of 0.7 EFT.

What You Need:

  • An Australian Association of Social Workers (AASW) accredited Bachelor Degree or entry level Masters Degree of Social Work which provides eligibility for membership of the AASW. Written evidence of eligibility for AASW membership for overseas qualifications is required. Demonstrated experience (aligned to positon description noting no reference to years of experience)

  •  Minimum 7 years experience as a qualified Social Worker Relevant Qualification (aligned to position description)

  • Current Victorian Drivers Licence

  • Relevant post graduate qualification or work towards Desirable: Dual Diagnosis qualifications or experience

  • Australian working rights
