We are One Peninsula Health.What You Will Be Doing The Transition Care Program (TCP) is seeking an experienced, innovative and flexible individual to work as a member of the TCP Case Management team. The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.TCP operates in both the Community and Residential Aged Care environments, offering scope for clinicians to enhance their experience along the care continuum.Please note the following;• Flexible working environment with options to do some work from home• Great varied caseload across inpatient residential and community based settings • Opportunity to develop/evolve a dynamic group of case managers• Great multidisciplinary close knit team What You NeedThe successful applicant will need to be flexible in terms of working across both residential and community areas. The Case Manager will need to demonstrate sound clinical judgement, assessment and practice skills, and have an understanding of the issues affecting patients with complex psychosocial needs and an interest in aged care issues. The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.
