ABOUT THE ROLE:

Grampians Health is inviting applications from driven and proactive candidates to join our dynamic team as a Service Access Worker. In this role, you will assist in coordinating a client's journey through Primary Care Services. This includes completion of the intake process; encompassing initial contact, registration, and initial needs identification for clients. The Service Access Worker ensures clients are offered the most appropriate services based on their needs and presenting issues.

The successful applicant will be part of the Primary Care team at Stawell Regional Health, including another Service Access Worker, and over twenty Allied Health professionals.

• You demonstrate excellent all-round IT and data entry skills including Microsoft Office, Excel and Outlook.

• You are able to demonstrate initiative, time management skills and flexibility.
• You have a positive, friendly, solutions focused attitude.
