This is a great opportunity for an individual to join the Community Services team, working as the Social Support Coordinator. In this role, you will be responsible for leading the Social Support team to provide a range individual and group activities according to clients individually assessed physical, social, cultural and spiritual needs and preferences, using a client focused care including the development of support plans utilizing wellness and enablement model.

Social Support Coordinator

Fixed Term Full Time – 80 hours per fortnight

Classification: Dependent on qualifications

Award: Victorian Public Health Sector (Health and Allied Services, Managers and Administrative Workers) Single Interest Enterprise Agreement 2016 – 2020

We are looking for someone with the following qualities:

  • Embrace and promote the values of MDHS – Genuine, Respect, Excellence, Accountability & Togetherness
  • Relevant qualification in Victorian HACC Program Manual; Cert IV Home and Community Care, or Enrolled Nurse with current AHPRA registration
  • Sound knowledge of Community Health Services
  • Ability to work independently and as part of a team.
  • Skills and expertise in leading small teams to achieve their optimal efficiency and effectiveness.

KEY SELECTION CRITERIA

Qualifications / Experience

  1. Experience in working in low socio economic communities with complex medical and social issues
  2. A relevant qualification as outlined in the Victorian HACC Program Manual.  This may include at least a certificate IV Home and Community Care, or Enrolled Nurse including a current AHPRA Practicing certificate
  3. Current driver's licence

Specialist Expertise

  1. Skills and expertise in leading small teams to achieve their optimal efficiency and effectivenes
  2. Sound knowledge of Community Health Services
  3. Recognition of possible challenges and strategies in order to work effectively in a community setting, to achieve best client outcomes
  4. Understanding and commitment to person-centred care and the principles of self-management

Personal Qualities

  1. Ability to work independently and as part of a team
  2. Ability to communicate and advocate in a timely and effective manner with clients, their families/ carers and other health professionals
  3. Computer skills
