About the role

An opportunity exists for a motivated individual to join the Ambulatory Aged Care Programs (AACP) as a Social Worker. The Social Worker is responsible for the planning and implementation of high quality Social Work interventions for a diverse consumer population.
The successful applicant will have well-developed time management and critical thinking skills, the ability to prioritise tasks and work under minimal supervision. Applicants should also possess excellent written and verbal communication and interpersonal skills and be able to work effectively within a team environment. A sound knowledge of computer based software applications including Word/Excel/database management systems is also required. This is a fulfilling and rewarding role, suited to those who enjoy a challenge and who demonstrate excellent problem-solving skills, whilst working in a supportive and collaborative team environment..

We are looking for

  • An approved tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers(AASW)
  • Demonstrated skills in assessment and intervention options for patients with diverse and complex psychosocial circumstances
  • Demonstrated understanding of person centred care in relation to adjustment to illness, disability or lifestyle changes
  • Demonstrated ability to work independently and as active member of an interdisciplinary team
  • Excellent interpersonal, written and verbal communication skills, including the ability to initiate and maintain effective professional relationships with a wide range of internal departments, community agencies and services
  • Demonstrated time management skills and organisational skills required to manage a clinical caseload and non- clinical responsibilities as required
