Social Worker – Community Health

  • Permanent Part Time - 2 days per week
  • Based at Healesville Hospital & Yarra Valley Health in the beautiful Yarra Valley, only 20mins from Lilydale.
  • $83,361.20 - $95,825.60 p.a. (pro rata for PT) + super + salary packaging

The Healthy Mothers, Healthy Babies Program provides flexible, outreach support to pregnant women who have difficulty accessing antenatal care services or who need extra support because they have greater health risks as a result of factors including:

  • social isolation or poverty
  • insecure housing
  • young maternal age (under 25yo)
  • cultural and linguistically diverse background or refugee status
  • substance use issues
  • mental health issues
  • family violence
  • a history of engagement with child protection services
  • Aboriginal and Torres Strait Islander descent
  • geographical barriers to accessing services

About you

To be successful in this role you will possess:

  • A current drivers’ licence
  • A current Working with Children Check
  • Eligibility for membership of the AASW and evidence of current Continuing Professional Education, as per AASW requirements
  • The ability to undertake complex psychosocial assessments and to formulate and implement appropriate evidence based interventions that are consistent with best practice and seek to identify risk
  • Excellent interpersonal skills including demonstrated flexibility, proficient negotiation and communication skills (written and verbal) to work collaboratively with vulnerable clients experiencing complex psychosocial issues.
  • Demonstrated experience in the field of family health and supporting people experiencing family violence
  • Demonstrated capacity to develop effective working relationships, collaborations and partnerships with a range of service providers, including maternity services, Child Protection, family services, mental health services and family violence services
  • Demonstrate a strong commitment to quality improvement and innovation.
